Winners must complete/sign a Prize Redemption Form at time of prize pick-up.
The winner is responsible for payment of all taxes on the prize and for any other expenses not specifically described herein.
The Loker Student Union will determine the winners and will issue final decisions on all matters relating to this survey.
Winners will be notified/announced by 5:00 PM on Monday, March 2, 2020.
Winners may pick up prizes on Monday, March 2, 2020 through Friday, April 3, 2020.
If a prize remains unclaimed 30 days or more after a winner has been notified, or if a winner fails to properly sign and return all forms described herein, the prize will be forfeited and awarded to an alternate winner.
By participating in the survey, responders hereby grant the Loker Student Union rights to edit, publish, use, adapt, modify, or dispose of any proper names, likenesses, photographs, and/or city/states without further consideration or payment.
